Incidents involving airline safety have been occurring in Japan in recent years despite government oversight. Every year for the past 5 years, the transport ministry has slapped flight operators with business improvement orders, issuing them to a total of 6 companies.
One of the orders was issued in July to Nippon Cargo Airlines. The company was operating without performing mandatory maintenance, and had been forging maintenance records. Transportation officials said the violations were serious and could have resulted in accidents.
Toho Air Service, the operator of a helicopter that crashed on Friday, also received an order in February.
Last November, officials conducted inspections at the firm after a different fatal helicopter crash. They discovered multiple violations of maintenance rules, including parts not being changed during repairs.
Transport ministry officials called the series of violations regrettable. They have vowed to continue working with operators to improve aviation safety.

Japanese Foreign Minister Taro Kono has set off on an 8-day tour of Central and South America.
Kono left Narita Airport, outside Tokyo, on Sunday afternoon. He will visit Ecuador, Peru, Colombia and Mexico before returning to Japan next Sunday.
Kono hopes to strengthen cooperation on maintaining free trade frameworks at a time of increasing protectionism.
Peru and Mexico are signatories to the Trans-Pacific Partnership. Colombia is showing interest in joining the free trade pact.
Japan aims to achieve the early launch of the TPP, and seek cooperation for the expansion of its membership.
Kono will become the first Japanese minister to visit Ecuador. He plans to offer support for disaster preparedness as Ecuador is prone to earthquakes like Japan.
In his talks with officials from the 4 countries, Kono will also seek support for Japan's stance on international issues, including those involving North Korea.

Japanese Prime Minister Shinzo Abe has indicated his intention to run in next month's leadership race of the main governing Liberal Democratic Party and seek a third term.
Abe spoke to supporters after visiting the grave of his late father, former Foreign Minister Shintaro Abe, in the city of Nagato, Yamaguchi Prefecture.
The prime minister said he has given his all for Japan and its people over the last 6 years. He said he intends to make the decision to continue with the heavy responsibility.

Police in Gunma Prefecture are trying to determine what caused last Friday's fatal helicopter crash. The local government aircraft went down in a remote mountainous region, killing all 9 men on board.
About 30 investigators are combing the mountains in Nakanojo town to inspect the wreckage and look for the victims' belongings.
The helicopter was on its way to check a new climbing trail when it crashed on Friday morning.
Police are trying to piece together what happened in the final 3 minutes of the flight.
Prefectural officials say the aircraft's GPS record shows it made a sudden, sharp turn around that time. The helicopter then flew at a low-altitude, just above the mountain slope at some points.
They say it was flying at about 20 to 75 kilometers an hour when it made the turn, but then accelerated to about 150 kilometers an hour and crashed horizontally into the mountain terrain.
Officials from the transport ministry's safety board are also carrying out interviews to determine the cause.
